If you require any vaccinations relating to foreign travel you need collect a travel form from reception at least eight weeks before the date of departure. Once you have returned the completed form and handed it back into reception, a nurse will use the information to determine what vaccinations are required for the country or countries you are visiting. They will then be in contact with you to arrange an appointment.
If you are travelling in more than six weeks time to a place where you think you will need a vaccination, please download the link below, complete the form, and hand into reception. Alternatively collect a form from reception.
The travel booklet below is also available from reception and has advice for your upcoming trip.

It is important that you make this inital contact with the surgery at least eight weeks beofre you travel because the nurse will have to order the travel vaccines as they are not a stock vaccine. Your appointment will need to be at least two weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
